Sample Submission

One form travels with every sample.

The Test Request Form tells the lab what to test, which methods to run and who receives the report. Fill it in on screen or by hand, then pack a printed copy in the box with your samples.

PDF, 3 pages. Fill it in right in your browser or in Adobe Reader.

How to submit

01

Fill it in

Type into the fields in your browser or Adobe Reader and save a copy, or print it and write clearly by hand. Anything not listed, whether a method, an organism or an analyte, goes in the Other box on page 3.

02

Name each sample once

The names you enter under Sample Identification are printed on the IAC report exactly as written. Use one line per sample, and attach a separate sheet if you have more than fit.

03

Pack it with the samples

Put a printed copy in the box with at least 20 grams of each sample; some methods need more. Sending the form ahead helps the lab prepare: email the saved PDF to the address printed on the form.

Not sure which method to request?

Tell us what the treated article is and the claim it needs to support. We will recommend the method, organisms and durability level before anything ships. Running many samples or a recurring program? We can build a request form tailored to it.
